However, the value of modern ultrasound systems extends far beyond tendons, ligaments and ovaries.

Exploring these alternative applications can help clinicians extract more diagnostic value from a tool they already have in their vehicle or clinic. With improved image quality, portability, and probe versatility, ultrasound has evolved into a highly adaptable, problem-solving tool that can be applied across a wide range of clinical scenarios.

In both ambulatory and hospital settings, ultrasound offers real-time, non-invasive insights that can significantly enhance diagnostic confidence. Expanding how and when we use this modality can support clinical decision-making, guide treatment, and ultimately improve patient outcomes.

This blog recaps some of the lesser known but highly practical ways ultrasound can be incorporated into everyday equine practice.

Where can ultrasound add further diagnostic value in equine practice?

Assessing and monitoring complex wounds

Supporting decision making in acute trauma cases

Characterising soft tissue swellings and masses

Examining ocular and retrobulbar structures when direct visualisation is compromised

Improving the accuracy of diagnostic and therapeutic procedures

Mindray Vetus EQ Veterinary Ultrasound Scanner

How can ultrasound refine the assessment of complex equine wounds?

Ultrasound can help define the depth and extent of a wound and identify any involvement of underlying structures.

Equine wounds can be deceptively complex, with seemingly superficial injuries often masking deeper pathology that is not immediately apparent on visual inspection or palpation alone. In these cases, ultrasound offers a valuable assessment beyond what is apparent to the naked eye. This includes revealing any potential involvement of bone, tendons, joints and bursae. It can also help detect the presence of foreign material, gas and pockets of fluid to guide therapeutics. In contaminated or chronic wounds, ultrasound is particularly helpful for differentiating granulation tissue from abscessation or necrotic areas.

Beyond initial evaluation, ultrasound can also be used to monitor wound healing over time, identify complications early, and support decisions regarding referral, surgical exploration, or continued conservative management.

Ultrasonographic assessment of a non-healing wound six weeks after initial injury, showing the presence of foreign material as hyperechoic foci (red arrows).

What can ultrasound add when fracture or periosteal injury is suspected?

Ultrasound can provide rapid, real-time assessment of cortical surfaces and adjacent soft tissues when radiography is unsafe, impractical or unlikely to answer the whole question. Emergency settings can be challenging environments which may not facilitate radiograph acquisition in a safe and effective manner due to factors such as patient location or positioning. In these situations, ultrasound can serve as a preliminary screening tool, providing important information about damage to the underlying cortical bone and associated soft tissue structures. Here, images can be viewed in real time to look for bony incongruencies and can offer both static and dynamic assessment of the affected limb.

In order to successfully evaluate bone contour and spot subtle abnormalities it is useful to first build a picture of normal anatomy by examining healthy animals or through comparison with the contralateral side of those that are injured.

Ultrasonographic assessment of the periosteum of the proximal humerus following a traumatic wound.

Ultrasound is also particularly useful for assessment of areas that can be challenging to radiograph such as the pelvis or proximal femur, where pathology can be masked by the large, overlying muscle mass.

Check out our handy guide for pelvic ultrasound which can be found here.

What can ultrasound reveal about a swelling or mass?

Ultrasound provides valuable information about the composition and internal architecture of a swelling. Swellings are a common presentation in equine practice, yet their underlying cause can be unclear based on palpation alone. Ultrasound helps to provide rapid clarification by allowing differentiation between solid and fluid-filled structures. This is particularly useful for identifying abscesses, seromas, haematomas, cysts, and neoplastic masses. It also offers valuable information regarding size, margins, internal architecture, and relationship of a mass to the surrounding tissues. Doppler can help in these situations by providing useful detail about blood supply and highlighting the presence, pattern, and degree of blood flow within or around a lesion. Such information can help guide fine needle aspiration or biopsy, improve diagnostic accuracy and reduce unnecessary sampling.

When does ophthalmic ultrasound add diagnostic value?

Ultrasound can be an extremely valuable tool when direct visualisation of ophthalmic structures is compromised. Conditions causing corneal opacity, severe periorbital oedema and blepharitis can all inhibit evaluation and prolong therapeutics.

Ultrasound allows the assessment of multiple ocular structures including the bones of the orbit, globe, lens, vitreous body, and retina, facilitating detection of a variety of conditions such as lens luxation, retinal detachment, intraocular masses, or vitreous haemorrhage. It can also aid identification of retrobulbar abscesses or masses contributing to exophthalmos.

In less acute settings, ocular ultrasound can also be used to help differentiate conditions such as iris cysts and melanomas, as well as providing additional information on the stage and location of cataracts.

As a non-invasive technique that can often be performed in the field, ophthalmic ultrasound plays an important role in prognosis, treatment planning, and referral decision-making.

How does ultrasound guidance support diagnostic and therapeutic procedures?

Ultrasound guidance allows the target, needle trajectory and surrounding structures to be visualised in real time, improving procedural precision and reducing iatrogenic risk.

Common applications include guided centesis of joints, bursae, and tendon sheaths, as well as abscess drainage, fluid aspiration, targeted injections, and tissue biopsies. This blog demonstrates that ultrasound is a dynamic, versatile tool that can be integrated into everyday equine problem-solving. Expanding its use does not necessarily require new equipment, but rather a broader perspective on how existing tools can be applied.

Through lateral thinking and by embracing both its practical and innovative applications, equine ultrasound enables veterinarians to enhance diagnostic confidence and ultimately deliver better patient care across a
